You can submit a Beneficiary Proof of Representation (POR) authorization request to inform the Centers for Medicare & Medicaid Services (CMS) that the Medicare beneficiary has given another individual or entity (such as an attorney) the authority to represent them and act on their behalf with respect to their case. An individual/entity with a Verified Beneficiary POR will be able to receive copies of all mail related to the case (e.g., the Rights and Responsibilities letter, the Conditional Payment Letter, the Demand letter, etc.) as well as receive identifiable health information or resolve any potential recovery claim that Medicare may have if there is a settlement, judgment, award, or other payment.
Note: A person with a verified Beneficiary POR authorization can perform all the following actions on the Case Information page for Commercial Repayment Center (CRC) and Benefits Coordination & Recovery Center (BCRC) cases:
