This User Agreement dialog appears when you, as the Account Manager (AM), begin your account setup. As indicated, it describes the terms and conditions for accessing a Coordination of Benefits (COB) Secure Website (COBSW).

All users must agree to similar language each time they log in to the Section 111 COBSW application.

1.  After reading, scroll to the bottom and click the checkbox to accept.

2.  To print, click Print at the top of the dialog.

3.  Click Continue to continue with your account setup.
